1. Go to the Holt Public Schools' Website. Click the PowerSchool icon.  

2. If this is your first time logging in, click on Forgot Username or Password? and follow the prompts. A change password link will be emailed to you.

3. Type your email address that is on file in HPS PowerSchool.

4. Enter your PowerSchool password.

5. Click sign in.

6. Click the tab with your child's name. 

7. Click Forms.

8. Click the Back to School Forms link

9. Carefully review each section. Items marked with an asterisk (*) must be answered in order to submit the form.

10. Carefully review each section and answer each required question. 

11. Use the dropdown menu to answer, when applicable.

12. If yes, please check the appropriate box and fill in the "Other" field if necessary. 

13. If you checked, "Other Allergy," please list all allergies. 

14. Continue to review and edit if needed. 

15. If yes, please check the appropriate box and in the "Other" field,  add health considerations that are not listed. 

16. Click box to acknowledge. This lets the district know that you have read and understand each policy.

17. Click, "Yes" if your student is tribally affiliated.

18. Right Click on the link and choose, "Open in a new tab," and complete the application.

19. Continue to carefully read each section and acknowledge by checking the box.

23. Type your name. 

24. Click the Submit button.
